Assisted Living

In addition to the amenities and services noted on our Overview page, we create individualized care plans for each of our assisted living residents. This custom care plan is based on a careful assessment conducted by one of our licensed staff nurses, and identifies all cares, monitoring, and activities of daily living (ADLs) that will be provided to the resident. Assessments are conducted prior to moving in to Guardian Angel Homes, every 90 days and any time a resident experiences a change of condition. Our comprehensive resident assessment considers resident needs in areas, such as:

  • Eating Assistance and Special Dietary Needs
  • Bathing
  • Dressing
  • Laundry and Housekeeping
  • Personal Hygiene
  • Mobility and Transfer Assistance
  • Night Needs
  • Communication Assistance
  • Appointment Scheduling, Community Access, and Transportation
  • Medication Assistance
  • Emergency Response and Assistance
  • Behavior Management (Anxiety, Wandering, Depression, Impulse Control, etc)
  • General Health & Vitals Monitoring
  • Socializing and Recreation
  • Coordination with 3rd Party Care Providers (Physical & Occupational Therapy, Hospice, etc)
